How to manage stress incontinence with bladder leakage? - Dr. Ravish I R

Urge incontinence, stress incontinence, leakage, if you take these all three, we have to find out what is the cause first to treat these patients, if there is any over active bladder, if there is a sphincter leakage causing leak or there is over flow incontinence. Increased frequency might be because of incomplete emptying of bladder or over active bladder. Urge incontinence, stress incontinence which is there, urge might be because of partial emptying of bladder and before going to toilet the sphincter is being very weakened. It is usually present in prostatic hypertrophic patient, or stress caused, when you cough there is an increased pressure, there is leakage of urine. So this stress incontinence is mainly present in females. If you take somewhat about 70% of the patient constitute the female and 30% the males. In females it is because of the shorter length of the urethra and only one sphincter being present. This may lead to stress. It might be due to repeated pregnancies in post menstrual age group. Normal vaginal delivery causes some amount of stress on the sphincter leading to stress incontinence. So evaluate the patient with an ultrasound, urine routine, urea creatinine and uroflowmetry. If the uroflowmetry shows disturbance in this please get a doctor or get a neurodynamic done for these patients. Urodynamics is nothing but to know how the bladder functions, i.e. an irritable bladder or a flaccid bladder or a overactive bladder. These things will give you what the disease is and then treating it will solve these problems.

There are two types, one is stress urinary incontinence and urge urinary incontinence. So by activating those muscles you can activate the incontinence. Urge incontinence is when we have the urge we feel like passing and that is very much related to psychology. In this we have to counsel the patient for a long time and teach them how to manage those situation and we have to tell them how to control it. That is also one way of hanging your lifestyle you should not feel like passing urine and immediately search for washroom to empty your bladder. Try to prolong the duration of holding the urine. By training it can cage your habit. It is also because of weakness or laxity of the muscles. If it is real laxity that cannot be helped with physical exercise that is the pelvic floor the Kegels exercise, that is why you have to g o for surgical treatment. We also have medicines to reduce the bladder irritability. That will help them t o hold the urine for a longer time. This is the best methods for incontinence. Incontinence can be better as the female ages. If the female ages the thickness of muscles also happens and when menopause happens there is lack of the routine hormones and that hormone is known to help he mucosa too help in controlling urination. Due to lack of estrogens there is chance of getting infections and hence w e also use creams that helps them to get over the problems of incontinence.

Pelvic Floor Exercises, Facts and Recovery

Pelvic floor exercises are also known as Kegel Exercises and are promoted as the starting point for building pelvic floor strength.

During pregnancy and after childbirth; your pelvic floor muscles are lengthened and weakened and as a result can cause incontinence. If you strengthen your pelvic floor, then you will be helping to prevent or reduce the severity of incontinence.

Where are Your Pelvic Floor Muscles?
The pelvic floor muscles are between your pubic bone at the front and your tailbone at the back. The pelvic floor muscles support the bladder, bowl and uterus. The openings from your bladder, your bowels and your womb all pass through your pelvic floor. They also maintain bladder and bowel control.

What Do Pelvic Floor Muscles Do?
Pelvic floor muscles provide support to the organs that lie on it. The sphincters give us conscious control over the bladder and bowel so that we can control the release of urine, faeces and flatus and allow us to delay emptying until it is convenient.

When the pelvic floor muscles are contracted, the internal organs are lifted, and the sphincters tighten the openings of the vagina, anus and urethra. Relaxing the pelvic floor allows passage of urine and faeces.

The pelvic floor muscles in women also provide support for the baby during pregnancy and assist in the birthing process. The muscles of the pelvic floor work with the abdominal and back muscles to stabilise and support the spine.

Symptoms of Pelvic Floor Problems
• Accidentally losing control of their bowel
• Accidentally leaking urine when you exercise, play sport, laugh, cough or sneeze
• Accidentally leaking urine when you cough or sneeze
• Accidentally leaking urine when you are being active or playing sport
• Pain in the pelvic area
• Constant urge to go to the toilet
• Finding it difficult to empty their bladder or bowel
• Accidentally passing wind
• Painful sex
• Poor sensation or leaking during sex
• A prolapse

I recently read this article about workout Peeing...Do you have any yoga exercises (I'm assuming Goddess pose would be one) to help strengthen the pelvic floor? Thanks in advance. I'm sure others would be interested in this topic as well. Namaste, Teresa

This is a great question Teresa. One of the most important things you can do to stop incontinence in its tracks is to strengthen your pelvic floor. In yoga we refer to this as mula bandha. I have a whole class that will help with that.



Let's do a little review of how this works. Inhale and on your exhale, think about drawing your pelvic floor up and inside of you like a drawstring bag. You can also imagine drawing your pubic bone and tailbone closer together.

As you mentioned, it is useful to practice pelvic floor lifts in different positions. Goddess pose is one of the more challenging positions to practice mula bandha because the muscles of your pelvic floor are actually spread quite wide in that position. That being said, it is a good position to challenge your pelvic floor muscles.

I like to practice in bridge pose because you have the help of gravity. Downward facing dog is another great place to practice. Mountain pose too. Practice Mula Bandha in a variety of positions to help strengthen your pelvic floor.

How To Do Kegel Exercises For Bladder Control

How to do Kegel Exercises for bladder control with Pelvic Floor Physical Therapist Michelle Kenway from Learn the 2 Kegel Exercises for controlling bladder urgency, storing bladder contents and stopping unwanted bladder leakage. Michelle also shows you how to feel your Kegel exercises to strengthen pelvic floor muscles and improving bladder control.

Finding Your Pelvic Floor Muscles

Your pelvic floor muscles sling like a hammock between you sit bones from side to side and front to back from your pubic bone to your tailbone.

Your pelvic floor muscles encircle your anus, vagina and urethra (urine tube). They can help you stop or slow the flow of urine when contracted as a test to see how they are working.

Two Types of Kegel Exercises for Bladder Control:

1. Long Slow Kegel Exercises - for bladder storage & support
2. Short Strong Kegel Exercises - stop bladder leakage with cough, sneeze, laugh or lift

A. Technique for Long Slow Kegel Exercises

*Sit with tall posture
*Squeeze and lift the muscles in and around your 3 pelvic openings
*Keep lifting and squeezing for 3-10 seconds (or even a few seconds if this is what you can manage)
*Breathe normally
*Lower your pelvic floor muscles back to starting position
*Relax and rest briefly before your next attempt

Start out with the number of exercises you can comfortably manage.

Repeat 8-12 Kegel Exercises, 3 times daily holding each exercise for 3-10 seconds.

B. Technique for Short Strong Kegel Exercises

*Sit with tall posture
*Squeeze and lift the muscles in and around your 3 pelvic openings with a quick brisk action
*Lower your pelvic floor muscles back to starting position
*Repeat up to 10 short Kegel exercises in a row

Start out repeating this exercise for that number of contractions that you can manage with the correct technique.

Practice these short contractions daily. Try to increase the strength of the contractions as you pelvic floor strength improves.

Kegel Exercises for Bladder Control

Use these quick contractions before and during every cough, sneeze or laugh. You need to contract your pelvic floor muscles before and during the activities that place pressure on your bladder causing bladder leakage.

Relax your pelvic floor muscles back to resting when you are not doing your exercises. Avoid walking or running with your pelvic floor muscles contracted as this practice can cause pelvic floor muscle tension and pelvic pain.

BackLoop Exercises for Urinary Incontinence and The Pelvic Floor
